Dental Insurance Information
If you would like for us to file dental insurance for your child please complete the fields below so we can contact your insurance company and confirm coverage. If you feel like you need help we are also happy to walk through your insurance over the phone.
*Some insurance companies require the subscriber's social security number to file claims. If that is the case we will call you and obtain that information over the phone in an effort to keep your sensitive information as secure as possible.
*Our office does not accept any form of Medicaid
